- All appointments must be scheduled in advance; please arrive 10–15 minutes early for paperwork and smooth service.
- A detailed treatment plan will be provided before any procedure; ask for clarification if needed.
- By attending your appointment, you consent to the treatments outlined in your plan; a signed consent form is required.
- Intraoral and facial photographs may be taken for diagnosis and records; all data is stored securely.
- Package charges apply only to included services; additional treatments will be billed as per clinic rates.
- Cancellations require at least 24 hours’ notice; program membership cannot be withdrawn once enrolled.
- Patients must provide accurate medical history and follow post-procedure instructions.
